Reconstitution maths: converting mg per vial to mcg per unit

The single calculation every benchtop researcher needs, plus the two mistakes that cause almost every error.

The core formula

Concentration is simply vial content divided by diluent volume. A 10mg vial reconstituted with 2mL of bacteriostatic water gives 5mg per mL, which is 5000mcg per mL.

On a standard 100-unit insulin syringe, one millilitre equals 100 units, so that same preparation is 50mcg per unit. Our on-site reconstitution calculator performs this conversion for you.

Mistake one: confusing units with millilitres

Syringe graduations are units, not millilitres. Ten units on a 100-unit syringe is 0.1mL. Treating the two as interchangeable produces a tenfold error in every downstream figure.

Mistake two: assuming displaced volume

At milligram quantities the solid displaces a negligible volume, so 2mL of diluent gives a 2mL solution. At higher masses, and for blends totalling 80mg or more, the assumption starts to drift and volumetric verification is worthwhile.
